This form does not take place of a consultation, it is only here to get contact information and information about sizing, color, placement and style of tattoo. I will be emailing you to schedule your appointment or consultation. Consultations are either done in person at the shop or done virtually. We will use this time to brainstorm your piece, grab photos of your body if needed for the design process, and go over pricing.

$100 non-refundable deposit

$150 minimum

$650 half day

$1,100 full day

Flat rate for flash designs. You can find my flash designs on my "Flash" Instagram highlight.

While it is legal to tattoo individuals under 18 in the state of Utah with parental consent, I have chosen to only tattoo people over the age of 18.

What are you wanting to get tattooed during this appointment? (Be specific unless you’re prepared for me to have full artistic freedom with your idea. To provide reference photos, please email those to me once you’ve submitted this form.) If I don’t receive reference photos and your response here is vague, I will draw up something based on my interpretation. On the day of your appointment, I can make small adjustments- but if it’s a full or major redraw, you’ll need to reschedule.

Where did you want it placed (including side of body if applicable) AND roughly what size?
